The Community of Hope Health Clinic (CHHC) is a medical mission that provides non-emergent medical care to Shelby County adults ages 19 - 64 who have no health insurance and whose household income is at or below 200% of the poverty level.

The Clinic opened its doors on October 16, 2008 after entering into a Memorandum of Understanding with the Shelby County Health Department for use of their building on Thursday evenings.

Other than three paid positions the Clinic is totally manned by volunteers.  Shelby Baptist Medical Center provides our diagnostic testing.  We also have physicians in the area who provide pro bono medical care for referrals that we make.

For Households with more than 8 persons, add $4,480 for each additional person.
*Alaska and Hawaii have different rates for HUD federal poverty guidelines.
These numbers above represent 100% of the Federal Poverty Rate. In order to get reduced or free services from some clinics, they use a sliding fee scale based on your income.When they use a sliding fee scale, the 100% rate can be different than 100%. In those cases, using for example a 200% federal poverty level, you will only need double the 100% number listed above to 200%.

I've been to this clinic.I needed medical care. This clinic was completely free. The staff was friendly. I waited less than 1 hour The correct phone number is 685-4154. You must call for an appointment they only see patients by appointment
