Medical services for the uninsured and low income persons. Sliding scale services provided.

We accept cash, checks (local banks ONLY), debit and credit cards. Insurance accepted only from United Health Care (State Health Care Benefit Plan Only), Cigna HealthCare (State Health Care Benefit Plan Only), Medicaid and Medicare. For other insurances, you must file an insurance claim directly to your insurance company for reimbursement. Receipts are provided to you to file your claim. Some of our service fees are based on a sliding scale. Unless arrangements have been made in advance, payments and any outstanding balances are expected at the time of service. Self-Pay: If you do not have medical insurance, you will be considered a Self-Pay patient. Self-Pay patients must pay for the medical services received at the time of the visit. Sliding Scale can be applicable for some services based on your income. Cash only for Travel Services.
