6:30 pm - 8:30 pm

Patient numbers are drawn randomly at 5:30pm.
The number of patients to be seen during the clinic is based on provider availability.

This is a walk-in clinic.

• Employment Physicals
• Patient Assistance Program Enrollment
• Chronic Disease Management
• Diabetes - Blood Glucose Testing
• High Blood Pressure Testing
• Others: Any Chronic Disease
