The Choctaw County Health Department offers clinical and environmental services to the public.
Certified copies of birth, marriage, divorce, and death certificates may be obtained from the health department.
Hours of Operation: 8:00 a.m.-5:00 p.m., Monday through Friday
Closed: 12:00 p.m.-1:00 p.m. for lunch
Customers can obtain most vital records from the Center for Health Statistics through their county health department in 30 minutes or less. Records may also be obtained through the mail in about 7-10 days. The cost is $15.00 for the first copy and $6.00 for each additional copy.

In those cases, using for example a 200% federal poverty level, you will only need double the 100% number listed above to 200%.
