You are required to bring the following to your eligibility appointment:
Proof of address - Documentation such as a receipt for rent, mortgage, lease, Arizona Drivers License or utility bill (electricity, water).
Proof of Identification Picture identification such as an Arizona Identification Card, Drivers License or U.S. Passport.
Proof of Income - Documentation of source of income for a one month period, (paycheck receipts, social security documents, unemployment receipt or pension).
Dependent Information (Birth date, Social Security number).
Payment for Services
CHCY accepts private insurance, AHCCCS, Medicare, as well as a sliding fee scale for eligible individuals. Co-payments and office charges are payable at time of service and can be paid with cash, personal check, debit or credit card.
