All prescriptions are required to be filled at MHC Pharmacies:
Marana Main, Clinica Del Alma, or Wilmot.***

Eligibility Requirements
You must...
1. Have a gross annual FAMILY income no
greater than 200% of the Federal Poverty Level
guidelines.
2. Must provide documentation of income and residency.
You will be assigned to our SFS program while
waiting for approval from AHCCCS.
Attention Dental Patients on SFS: 50% of your
dental fee must be paid at time of service

In order to get reduced or free services from some clinics, they use a sliding fee scale based on your income.When they use a sliding fee scale, the 100% rate can be different than 100%. In those cases, using for example a 200% federal poverty level, you will only need double the 100% number listed above to 200%.
