The Forestville Wellness Center (FWC) is the integrative health services site for West County Health Centers (WCHC) in Sonoma County, California. To broaden the medical and health options for WCHC patients, services at the Wellness Center include traditional, Individual Patient-Provider Appointments as well as an ever growing array of Group Visits.
All services are available for patients of West County Health Centers and offer a wonderful opportunity to create and maintain a healthier lifestyle. Ask your Care Team or call today to see if they have consultations or group visits that can help you on your journey to wellness.

As Scheduled - Holistic Health Consults and Wellness Groups
Group Visits is a term used to describe a wide variety of visits designed for groups of patients, rather than individual patient-provider appointments. Group visits have been found to be a highly effective means of health care delivery that can assist in meeting not only the patient's physical and medical needs, but also provide educational tools and social and psychological support.
Some groups are led and facilitated by WCHC medical staff and some groups are facilitated by instructors from community partners such as The Ceres Community Project and the Northern California Center for Well Being. Regardless of the facilitator, all groups are overseen by West County Health Centers medical providers.
Some of the groups are structured as a series of sessions and intended for the participants to begin together and stay together for a designated number of weeks (from 4 - 8 weeks), while other groups are one session only, yet repeated at different times throughout the year. Still other groups, such as Yoga, Stress Management, and Healthy Movement are offered on an ongoing, weekly basis.
The Forestville Wellness Center offers any patient of the West County Health Centers the unique opportunity to engage in these Wellness Group Visits which offer education, support, and tools for lifelong, healthy behavioral changes.

These numbers above represent 100% of the Federal Poverty Rate. In order to get reduced or free services from some clinics, they use a sliding fee scale based on your income.When they use a sliding fee scale, the 100% rate can be different than 100%. In those cases, using for example a 200% federal poverty level, you will only need double the 100% number listed above to 200%.
