This clinic offers comprehensive primary health care. This clinic's staff includes board certified physicians, family nurse practitioners, a certified nurse midwife, registered nurses, a registered dietician, clinical psychologists, and mental health counselors.
This clinic has a special mission to provide primary health care to uninsured and low-income residents of southwest San Diego County who can't afford the full cost of essential health care services. This clinic accepts Tricare, Medi-Cal, Medicare, Healthy Families, AIM, CHAMPVA, and other insurances, and uninsured patients can apply for a reduction in costs for essential services; no one is turned away for lack of funds.

Registration
When you arrive at our clinic, you will receive a Phreesia pad at check-in. With Phreesia you can review and update your demographic and clinical information, as well as pay any balances, privately on the pad. You will also receive a PatientLink tablet to complete your personal medical history.
We know that convenience is important to you. That is why we make our registration on-line before your visit. By completing your registration before your visit, you can significantly reduce your wait time in our lobby.
Also, please bring with you the following:
Driver License or other valid photo identification
Proof of income, if uninsured
Social security number
Insurance I.D. card
Medicare and/or Medi-Cal card(s), if applicable
Employer information: name, address and telephone number
We accept a broad range of payment and insurance plans, including Medi-Cal, Medicare and private insurance. Please be prepared to pay your co-payment at the beginning of your appointment.
If you do not have insurance, you can apply for our sliding fee discount program. Through this program we offer visits as low as $35 (based on your income and family size). For more information, please inquire during your visit or call (619) 429-3733.

These numbers above represent 100% of the Federal Poverty Rate. In order to get reduced or free services from some clinics, they use a sliding fee scale based on your income.When they use a sliding fee scale, the 100% rate can be different than 100%. In those cases, using for example a 200% federal poverty level, you will only need double the 100% number listed above to 200%.
