Patients are assigned to a physician in ChapCare's network and work closely with a team to ensure comprehensive care, regardless of ability to pay. Our fully integrated system of care improves health outcomes, reduces health care disparities, and encourages prevention and healthy living.

ChapCare believes that behavioral health services have a positive impact on the quality of our patient's lives and their families. To this end, ChapCare offers short term individual counseling and group behavioral health services to the patients it serves.

In addition to the high-quality, comprehensive health care services provided at our four health care center locations, ChapCare attends events and health fairs at various locations in the San Gabriel Valley, offering a wide range of health, wellness and preventive services.
