Community Care offers a sliding fee scale program for people who are uninsured or under-insured. 

The sliding fee scale is based on your family size and income.

This allows you and your family to pay a reduced fee for covered medical and dental services at Community Care of West Virginia locations.

Community Care of Flatwoods offers an array of medical services including comprehensive primary care and chronic disease management. 

These services include:

• Physical and DOT exams
• Sports physicals
• Well-woman exams
• Acute & Chronic illnesses
• Chronic disease management
• Vaccinations & immunizations

For acute, non-life-threatening illnesses and injuries, CareXpress is located two doors down and is open seven days a week.
