Community Health Services of Union County, Inc. (CHSUC) has provided community-based health screening clinics for residents throughout Union County where the greatest disparities lie.
These screenings include Blood Pressure, Blood Sugar, Hemoglobin, A1c and Cholesterol (HDL, LDL, Triglycerides). We offer American Diabetes Association-approved diabetes education, performed by our full-time Certified Diabetes Educator, and counseling by licensed professional nurses (RNs).
Community-based immunizations for Seasonal Flu are also provided. This vital program is community-based to provide access and delivery to patients who otherwise would not have accessibility to these services in a for-fee clinic.
As the 7th fastest growing county in the United States, Union County has many residents without jobs due to the failed economy, without insurance, and no accessibility to affordable medical care. Our programs target the uninsured, low income, elderly and single parents, of which statistics show 80% (exclusive of the flu program) have no insurance, Medicare nor Medicaid.
Public transportation and affordability of healthcare services continue to be an issue for underserved Union County residents which prompted CHSUC to choose clinic sites (15 locations throughout the county) where the greatest accessibility is achieved. These clinic sites were chosen on the basis of data obtained on income, age, uninsured, employment status and ethnicity.

*Alaska and Hawaii have different rates for HUD federal poverty guidelines.
These numbers above represent 100% of the Federal Poverty Rate. In order to get reduced or free services from some clinics, they use a sliding fee scale based on your income. When they use a sliding fee scale, the 100% rate can be different than 100%. In those cases, using for example a 200% federal poverty level, you will only need double the 100% number listed above to 200%.
