The Community Health Clinic (CHC) provides basic medical care to the working poor and uninsured citizens of Licking County. We do not have a dental clinic. Click on About Us for more information.
Appointments are made by phone for day of clinic only.

A $5.00 donation is requested.
Hours:
Monday: (Medical) 5:30-7:30PM
Wednesday (Medical) 12:30-3:30PM
Except 2nd Wednesday, 5:30-7:30PM
Thursday (Nurse and social worker only) 10:00-12:00PM

In those cases, using for example a 200% federal poverty level, you will only need double the 100% number listed above to 200%.
