IF YOU DON'T HAVE INSURANCE
Charter Oak Health Center offers a sliding fee scale for our patients who don't have insurance. This scale is based on your income and family size. When you come for your visit, you will be asked to fill out an application and provide proof of your income, your address and your identification. You will be asked to pay the sliding fee amount that has been determined for you at the time of your visit.
La Casa de Puerto Rico serves the Latino community of Hartford by helping with the social, economic, and political status of Puerto Ricans in Connecticut through the many programs that are offered. The programs include: the development of living for families from low income households, activities for youth, services for the elderly, and help with civil rights.
Since this is a sliding fee scale clinic, we have provided the Federal Poverty Guidelines below. Visit the La Casa De Puerto Rico website listed above to see what the level is needed for free care.
