They offer a Sliding Fee Discount Program for individuals and families who do not have insurance or whose income is below 200% of the Federal Poverty level. These patients must provide proof of their income (W-2's, Individual Tax Return 1040, copy of entitlement checks, last 2 paystubs) at time of visit in order to be eligible for the program. The health centers fees will be discounted will be based on the patients family size and income, as well, the Federal Poverty Level Guidelines in conjunction with the Sliding Fee Schedule determines the patients discount. They make sure their patients will not be turned away based on their inability to pay, and the patients ability to pay is determined by your annual income and family size.
