Our doctors, other health care providers and professional support staff will tailor your care to meet your needs. Azalea Health St. Augustine just moved! Visit us at our new location at 105 Whitehall Drive, Suite 109. Across from the rear entrance to Walmart.

In order to get reduced or free services from some clinics, they use a sliding fee scale based on your income.When they use a sliding fee scale, the 100% rate can be different than 100%. In those cases, using for example a 200% federal poverty level, you will only need double the 100% number listed above to 200%.

    Im 23 been diagnosed with scoliosis and my spine isnt fused to my tail bone causing massive pain. Have Bradycardia and have a pacemaker need a cardiologist and i need birth control. I have no insurnace and no job we are living with his mom but im responsible for my own medical bills. I do not qualify for medicaid becausw i do not have children.
