Our Primary Care Program is focused on Insured and Uninsured people in our Community. We provide a medical home offering acute and chronic care for illnesses, and preventative health care as well. Our patients range from newborn to the elderly. We currently accept AVMED, FLORIDA BLUE, and SEVERAL MEDICAID MMA.
IMPORTANT NOTICE FOR OUR CLIENTS
There will be a $10.00 minimal fee for *primary care visits unless you have insurance.
*Family Planning and Immunization clinics are excluded from the minimal fee.
The Florida Department of Health in Bradford County offers the following Primary Care services.
Medical History
Physical Exam
Screening
Limited Prescription Assistance
Health Education
Well and Sick care visits
Medical Follow-ups when a physician or other medical provider is available.
Please Note ** All programs in adult health are based upon funding availability.
Physicians are on call 24 hrs/day, 7 days/week for emergencies.

In order to get reduced or free services from some clinics, they use a sliding fee scale based on your income.When they use a sliding fee scale, the 100% rate can be different than 100%. In those cases, using for example a 200% federal poverty level, you will only need double the 100% number listed above to 200%.
