Who is Eligible?
Children 3 year old to age 20 receiving Medicaid
Patients over the age of 20 receiving Medicaid may qualify for emergency dental care
Emergency dental care is only provided during normal hours of operation
Cash paying clients (Individual with no insurance)
You must bring the following to your child's appointment:
Your child's Social Security Card and Medicaid Card;
Your Driver's License or other form of photo ID.
If you are not the parent, you will need to bring proof of guardianship or custody.
Immunization Services
Florida Department of Health in Jefferson County administers a variety of immunizations to infants, children and adults to protect them against certain disease causing bacteria and viruses that result in doctor's visits, hospitalizations and even death.
Immunizations services are provided free of charge to all children through the age of 18. This includes all childhood immunizations.
Clients should bring their most current immunization record (if available) and children should have a parent/legal guardian present for immunization consent.
Immunizations are available on a walk-in basis or you can call for an appointment .
