Eligibility and Cost of Services
Florida Department of Health in Levy County strives to make services available to as many people as possible based on our available resources.  Eligibility and or fees for services vary by program.
We accept BlueCross and Blue Shield, Medicare, Avmed and Medicaid Managed Care Plans including Staywell, Prestige, and Sunshine.  We accept the following Medicaid Managed Care Plans for children:  MCNA, Liberty, Argus, United and Dentaquest.

    Child and adult primary care
    Immunizations (children up to 18 years)
    Flu, pneumonia, DTAP, Shingles, and HPV vaccines for adults
    Health education and counseling
    Smoking cessation program referral
    Hepatitis screening
    Blood pressure screening
    Epilepsy and insulin medication program for eligible adults
    Breast and cervical cancer screening for uninsured women 55 to 65
    Dental Services for children and extractions only for adults

Diagnostic Services: Exams and XRays
Preventative Services: Cleanings, flouride treatments and sealants
Restorative Services: Resin-based fillings, stainless steel crowns and space maintainers
Basic Oral Surgery: Extractions
