Do you qualify for assistance paying for your health insurance, or for Medicaid or CHIP?
If you are applying for Medicaid or CHIP (Children's Health Insurance Plan) then you may do so anytime you choose. There is no open enrollment for these two types of insurance. And the Affordable Care Act Certified Application Counselors at Azalea Health can help you apply for Medicaid and CHIP. Or, if you don't qualify for either Medicaid or CHIP then the Certified Application Counselors can help define the insurance options available to you through the Health Insurance Marketplace.
Regardless of your needs, the Certified Application Counselors are here to help you, with no strings attached.
Azalea Health's Outreach and Enrollment team is comprised of Certified Application Counselors (CAC's). Our counselors received federal training about the new legislation, what it means for you and how to best advise you regarding the various options available to you. And they'll assist you with getting your application submitted and with any questions you may have.

For Households with more than 8 persons, add $4,480 for each additional person.

These numbers above represent 100% of the Federal Poverty Rate. In order to get reduced or free services from some clinics, they use a sliding fee scale based on your income.When they use a sliding fee scale, the 100% rate can be different than 100%. In those cases, using for example a 200% federal poverty level, you will only need double the 100% number listed above to 200%.
