The Florida Department of Health in Brevard County provides preventive dental services to children that meet the following criteria:
Children who are under 18 years old and do not have insurance can be seen through the uninsured dental program if they go through intake and qualify financially.
Children ages 0 to 18 receiving Straight Medicaid, DentaQuest, Liberty, CMS, United Healthcare, Argus, Dental Health and Wellness insurances.
Must have Birth Certificate and/or court paperwork appointed legal guardianship for each eligible child.
Adults over the age of 20 with Medicaid needing emergency dental care may contact 321-637-7300 Monday through Friday 8 a.m.-4 p.m.
Adults without Medicaid must qualify financially through intake. Once intake is completed call 321-383-2784 or e-mail Brevard.AdultDental@flhealth.gov to be added to the waiting list. This is not an emergency clinic and wait time for an appointment will fluctuate. This program is staffed by VOLUNTEERING dentists.

Family Planning
The Florida Department of Health in Brevard County determines eligibility for women of child-bearing age who are interested in birth control methods. Clients may be eligible for a Florida Medicaid Family Planning Waiver or a sliding fee scale based on income.
You may elect to pay full fee instead of going through our eligibility screening process.
Pregnancy Tests are available for women in need of a test for eligibility determination purposes. A $7.00 fee applies.

Brevard County resident.
Family Income below 100% of the Federal poverty guidelines.
Currently not covered by Medicaid, Medicare, private health insurance, military health benefits or other insurance.
Currently not enrolled in a similar medical program such as: hospital primary care, community health clinic, health alliance, etc.

Prenatal Care
All Brevard County residents are eligible for prenatal services. The Clinic Eligibility Staff can help you with your application for Presumptive Eligibility for Pregnant Women Medicaid. Please call for an intake appointment:

The Florida Department of Health in Brevard County Maternity program also offers a self-pay package Global Payment Option for those who do not qualify for Presumptive Eligibility for Pregnant Women Temporary Medicaid or other Medicaid options. The Prenatal Package is $1900. This option includes routine prenatal office visits and routine prenatal laboratory tests, ultrasounds and physician delivery charges.
A payment plan can be arranged. If you receive Medicaid coverage for the full term of your pregnancy, you may qualify for a partial or full refund on payments made toward this Prenatal Care Package.
WIC
To be eligible you must:
Be pregnant
Be breastfeeding
Have recently had a baby
Have an infant or a child under 5 years old
Have a low or moderate income
Have a special need that can be helped by WIC foods and nutrition counseling
Income Eligibility Requirements
You must meet the income eligibility requirements or be currently receiving Medicaid, temporary cash assistance (TCA), or food stamps. However, you do not have to be on a public assistance program to qualify for WIC.
What to bring to the WIC certification visit:
Bring EACH family member who is applying to receive WIC services.
Bring proof of ALL income for everyone in the household (before taxes and deductions are taken out). Income includes such things as: salaries, child support and alimony, foster care payments, interest withdrawn, unemployment compensation, and military earnings; OR, if you receive Medicaid, Temporary Cash Assistance (TCA), or Food Assistance, you are income eligible for WIC. (If you do not receive Medicaid or TCA, but other members of your household receive these benefits, you may be income-eligible for WIC.) WIC staff can verify your enrollment in these programs. However, it is helpful to bring the current "Notice of Case Action" letter, which is proof of eligibility for one or more of these programs.
Bring ONE proof of where you live (no P.O. boxes). The document must be current and show your name and address. Items that are allowed include a utility bill, bank/insurance statement, or driver's license. You must be currently living in Florida, but you don't have to be a U.S. citizen.
Bring ONE proof of identification for you and ONE proof of identification for any infant or child applying for WIC, such as a birth certificate, driver's license, crib card, military ID, photo ID, Social Security Card, or hospital record.
For EACH woman, infant, and child applying for WIC, you will need:
A recent height and weight measurement (no older than 60 days), if available, and
Hemoglobin or hematocrit blood test results (not required for infants under 9 months), if available.
Ask your doctor or health clinic to fill out this health information on a WIC Medical Referral Form or, the WIC office will help you get this health information.
Bring the Social Security Number for each person applying for WIC, if available.
Bring the immunization (shot) record for each child.

In order to get reduced or free services from some clinics, they use a sliding fee scale based on your income.When they use a sliding fee scale, the 100% rate can be different than 100%. In those cases, using for example a 200% federal poverty level, you will only need double the 100% number listed above to 200%.
