Services at this facility are extensive and include Medical Diagnosis, Treatment of Illnesses, Immunizations, Physical Examinations, Screenings for Asthma, Hypertension, Diabetes, Tuberculosis, HIV and other Sexually Transmitted Diseases, Nutritional Guidance, Health Education, Laboratory Testing, Pharmacy, Family Planning and Birth Control Counseling, Gynecological Services, On-Site X-Ray, Mental Health Counseling, Hospital Coverage, and 24-Hour Physician Coverage.
Rural Health Care, Inc. d/b/a Family Medical & Dental Centers (FMDC) is a private non-profit community and migrant health center established in rural Putnam County, Florida in 1977. FMDC is also a federally qualified health center (FQHC) and operates in areas designated as Health Professional Shortage Areas and or Medically Underserved Areas or Populations. FMDC physicians are Board Certified or eligible in Family Practice, Internal Medicine and/or Pediatrics.
