Genesis Community Health's Caldwell Clinic provides free healthcare to those who are uninsured and have a total annual income is less than 200% of the current Federal Poverty Guidelines.

Appointments are required. Call (208) 455-1143 to schedule.

Services include:

• Non-emergency family medicine
• Chronic disease management, including diabetes, hypertension, COPD, depression, and asthma.
• Medication assistance
• Basic dental services (extractions and fillings) and restorative dental hygiene
• Counseling services for minor addictions, depression, relationships and other challenges.
• Healthy Transformation Wellness Classes
• Connections to organizations that provide housing, food, and employment assistance programs

Clinic Hours: (by appointment)

• Monday 8:30 AM to 5:00 PM
• Tuesday 12:30 PM to 9:00 PM
• Thursday 8:30 AM to 5:00 PM
