Community Health Center (FQHC), offer a sliding fee scale to those who apply and are eligible.

The Good Samaritan Health and Wellness Center is a not-for-profit, Community Health Center, which provides health services to our community regardless of ability to pay or insurance status.

    Medical Care, including some specialty care
    Dental Care
    Eye Examinations and Glasses
    Mental Health Counseling
    Imaging and Laboratory Services
    Pharmacy
    Healthcare Education
    Social Services
    Vaccines

Hours of Operation:
Monday, Tuesday, Wednesday, and Friday: 8:30 a.m. to 5:00 p.m.
Thursday: 8:30 a.m. to 8:00 p.m.

For Households with more than 8 persons, add $4,480 for each additional person.
*Alaska and Hawaii have different rates for HUD federal poverty guidelines.
These numbers above represent 100% of the Federal Poverty Rate. In order to get reduced or free services from some clinics, they use a sliding fee scale based on your income.When they use a sliding fee scale, the 100% rate can be different than 100%. In those cases, using for example a 200% federal poverty level, you will only need double the 100% number listed above to 200%.

    I have restless leg syndrome horribly, so bad that it even goes into my arms and I'm completely out of my medicine. Without it, I don't sleep at all.
