Our patients are between the ages of 18-64 and have no health insurance. Some exceptions may apply; call and ask us.

Become A Patient
First, call us at (336) 835-1467 for an appointment to become qualified (this is before you receive appointments for medical care).
When you come, bring:
A. Identification
State-issued photo identification showing your current address.
Additional proof of where you live (like a power or water bill).
Any other government-issued identity cards.
B. Financial Information
A list of everyone in your household 'their names, ages, and relationship to you.
Proof of household income:
copy of 2009 tax return;
W-2s;
stubs from paychecks or benefit payments;
award letters for child support, Social Security, unemployment assistance, veteran's benefits, or Work First;
copies of alimony checks;
a description of any help you receive from other people, ministries, or agencies.
C. Medical Information
The bottles for any medications, supplements, or vitamins you take or have recently taken.
Any other medical records you might have.
A list of your health questions or concerns.

In order to get reduced or free services from some clinics, they use a sliding fee scale based on your income.When they use a sliding fee scale, the 100% rate can be different than 100%. In those cases, using for example a 200% federal poverty level, you will only need double the 100% number listed above to 200%.
