Appointments
Patients are seen on an appointment and walk-in basis. Please call during regular business hours to schedule an appointment.
Fees and Payments
Payment is expected at the time of service unless prior arrangements have been made. Bay Clinic accepts checks, cash, Visa/Mastercard, private insurance and Medicaid/Quest.
Please remember that no insurance company covers all dental costs. It is your responsibility to pay any deductible or any other balance not paid by your insurance company. With private insurance, please be prepared to pay your copay at each visit.
All of our services are available to you whether or not you have health insurance. If you qualify for QUEST (Hawaii's State Medicaid Managed Care Plan), we can help you get set up and covered. If not, you can be seen at any of our four clinics for primary care, behavioral health, or dental health.
If your income is at or below the federal poverty guidelines, you may qualify for a discount on the cost of your medical care. In order to qualify for a discount, you must bring with you to your appointment verification of income. This can be:
Most recent paycheck stub
Most recent welfare benefit statement
Most recent food stamp benefit statement
Most recent unemployment or workers comp benefit statement
Most recent tax return
Any other document that will prove how much income you receive.

These numbers above represent 100% of the Federal Poverty Rate. In order to get reduced or free services from some clinics, they use a sliding fee scale based on your income.When they use a sliding fee scale, the 100% rate can be different than 100%. In those cases, using for example a 200% federal poverty level, you will only need double the 100% number listed above to 200%.
