Located in Steinmetz High School, it is the organization's first school-based health center, serving the communities of Belmont-Cragin, Dunning, and Montclare. The site provides primary care services to people of all ages.

PCC accepts all patients regardless of their ability to pay for services. They accept major health plans, Medicaid/All Kids and Medicare, and they offer a sliding fee discount program based on total family income and size to individuals who lack health insurance. Spanish speaking medical and support staff are available at each PCC on-site.
