Park DuValle is an independent not-for-profit community health center. We have served Louisville's health care needs for more than 40 years.
The center's services are affordable, comprehensive and of high quality. As a federally qualified health center surveyed and licensed in the Commonwealth of Kentucky as primary care centers, Park DuValle Community Health Center strives to adhere to the highest quality standards of patient care, regardless of insurance or ability to pay. These numbers above represent 100% of the Federal Poverty Rate. In order to get reduced or free services from some clinics, they use a sliding fee scale based on your income.When they use a sliding fee scale, the 100% rate can be different than 100%. In those cases, using for example a 200% federal poverty level, you will only need double the 100% number listed above to 200%.
