The Lafayette Community Health Care Clinic is a non-profit organization that provides quality outpatient health care
for the eligible working uninsured and develops and provides programs to address community health care needs through collaborative partnerships.
The primary goal of this effort is to make basic health care services accessible to lower income working Lafayette
Parish residents so that they will remain healthy enough to maintain their jobs, their homes, and their
independence.

HOW TO MAKE AN APPOINTMENT
You must have an appointment to be seen by a doctor or dentist at the Clinic.
The appointment line is only answered on Tuesday and Thursday mornings between 10:00 am - 12:00 pm for that evening's Clinic. Appointments are taken at this time on Tuesday for general medical for that night and on Thursday for general medical and general dental (tooth extractions) for that night. All other apointments are made in person.

Eligibility Screening Times: (effective August 1st 2014)
Wednesday Afternoons:
11 am to 1 pm
Tuesday and Thursday Evenings:
3:00 pm to 6:00 pm
Patients must have the following qualifications:
Resident of Lafayette Parish
Working or actively seeking employment
Have no health insurance
Low to moderate income.Total income for all members of the household must not exceed 200% of the Federal Poverty Guidelines.
Patients Not Eligible Have:
Medical Insurance, Medicare, Medicaid, or Disability benefits
Job-related injuries involving Workers Compensation
A pending medical-based lawsuit
Not working
DOCUMENTATION NEEDED
Most recent pay stub (If self employed,
ledger for past three months)
Proof of income for all members of household
(including child support, alimony, SSI)
Most recent 1040 income tax return
Proof of residence in Lafayette Parish

LAFAYETTE COMMUNITY HEALTH CARE CLINIC offers patients a variety of health services including:
General medical services
Standard gynecological services
Preventative health care education
Pharmaceutical services (Prescribed medications at no charge. The Clinic does not provide controlled drugs or contraceptives.)
Dental services (Evaluations, dental hygiene and education, extractions, and prescriptions as needed for infections)
Eye Clinics (for diabetic and hypertensive patients)
General nutrition clinic
Lab and X-ray referrals (hospitals)

In order to get reduced or free services from some clinics, they use a sliding fee scale based on your income.When they use a sliding fee scale, the 100% rate can be different than 100%. In those cases, using for example a 200% federal poverty level, you will only need double the 100% number listed above to 200%.
