Legacy of Care Health Center provides comprehensive healthcare services to the uninsured and underinsured. Our patients demonstrate high levels of responsibility and accountability. Although our services are available to the general public by appointment, patients must provide documented proof they need our services.

Services provided at the clinic and in the community include:  
    Basic primary care: Physical Examination                                                       
    Prescription Medications
    Laboratory Testing
    Diagnostic Testing
    Specialty Referrals
    Cancer Screening (Cervical, Breast, Colorectal, Prostate)
    Screening in Osteoporosis, Peripheral Artery Disease
    Preventive Dental Care
    Patient Education

Patients must demonstrate a genuine need for free health clinic services. Eligibility requirements include:
    Twenty-one (21) years and older
    No public or private health insurance.
    Monthly gross income at or below 200% of Federal Poverty Level.
    If employed:  Two (2) paycheck stubs as proof of income.
    If unemployed:  Notarized Letter of Support
    Resident of Duval County.
    Proof of denial of coverage by Medicaid
