i beleive my fiance has the start of diabetes his sugars are always high even all the way to 546 first thing in the am

My son is 21 and I am concerned about his overall health. He has no health insurance and despite my begging he won't go to the ER because he feels they'll just do anything to get him out the bed and out the door. My son complains nearly daily now about not feel good in general, pain in his head and legs that some times makes him cry. I've tried everything under the sun, nothing works for long

Thus far hes tried, better diet and daily vitamin + adequate amount of water. OTC pain meds, Pain cream, rest, Epson salt soaks in the bath, stretching, and more. I've helped him monitor what he ate and caffeine intake. Nothing seems to correspond with the systems. He's only 21, would a labor intensive job be capable of causing someone so young this much distress. please advise TY
