Monday, Tuesday, Thursday & Friday: 8:30am to 5pm
Wednesday: 8:30 to 8pm
Saturday: 8:30 to 12pm
Services: OB/GYN, Well-Child Visits, X-Ray Services, Reproductive Health, Prescriptions, Lab Services
Patients at this clinic are eligible to apply for our Sliding Fee Program. The program is funded in part through federal grant monies and offers discounted fees to qualifying patients, because prevention health care should be affordable to everyone regardless of income.

Patients of Downriver Community Services are eligible to apply for our Sliding Fee Program. This program is funded in part through federal grant monies and offers discounted fees to qualifying patients. Preventive health care should be affordable to everyone regardless of income.
The following documentation of all that applies to your household is required to determine eligibility:
Last 2 pay stubs of all working household members
Proof of any other household income (child support, worker's comp, unemployment, rental income, pension, etc.)
Social Security New Benefits Income Statement or other proof of current S.S. income (if applicable)
Most recent Federal Income Tax return
Birth certificates of all children 18 and under to be covered by the program
Birth Certificates for all women 19 - 44 (Plan First application required, unless already enrolled)
Social Security Card of all applicants
Medicaid or Medicare card (if applicable)
Covered Services
Services performed at our facilities and prescriptions written by our physicians will be discounted under the terms of this program. Discounts do NOT apply to charges imposed by hospitals, laboratories, or other physicians. The program will cover select lab tests when performed by a physician at our facilities. A list has been included with your handbook. All other lab tests are the responsibility of the patient. If you have questions about the cost of lab tests not covered by the program, please discuss them with your doctor or the laboratory technician. Our nursing staff may be able to contact the lab to obtain a price for you. In many cases the lab will discount their fees for our patients.

These numbers above represent 100% of the Federal Poverty Rate. In order to get reduced or free services from some clinics, they use a sliding fee scale based on your income.When they use a sliding fee scale, the 100% rate can be different than 100%. In those cases, using for example a 200% federal poverty level, you will only need double the 100% number listed above to 200%.
