We see the uninsured residents of Michigan that are at or below 250%
FPL. We are free and even provide medications and dental care, also free
of charge.
Our Mission is to provide free primary medical care, women's health care and dental services to low income, uninsured and underinsured families of our neighborhood community in a caring compassionate atmosphere regardless of race, religion or creed.
Hours: Mon - Fri: 8:30 am - 4:30 pm
To become a patient of the Gary Burnstein Community Health Clinic you must be:
18-65 Years old
an Michigan resident

If you meet the above criteria and would like to become a patient please bring the following to the GB Community Health Clinic:
Supply your most recent financial information as one of the following
- 1040 or 1040ez (tax return)
- Proof of non-filing status from the IRS For those who did not file taxes last year or have no income.
- The IRS can be reached at 1-800-829-1040
- Supply a valid, government issued, picture ID (drivers license, state ID, prison ID, or green card)
- Letter of Denial from Medicaid
- Completed Patient Registration
- Completed Patient History
- Completed Patient Consent Contract
- Completed Release of Medical Information

Dental Clinic
Like all things at the GBCHC services provided by the dental clinic are free of charge to our patients.
Unfortunately due to a low volume of local dental resources and a high volume of needy teeth there is a waiting list for the dental side of the Gary Burnstein Community Health Clinic.Dental Volunteer

Mission Statement
In 1997, after working 20 years as a cardiologist, Dr. Gary Burnstein began providing care for patients at a homeless shelter in Pontiac, Michigan. Dr. Burnstein passed away in 2003, but friends, family, and community members

For Households with more than 8 persons, add $4,480 for each additional person.
*Alaska and Hawaii have different rates for HUD federal poverty guidelines.
These numbers above represent 100% of the Federal Poverty Rate. In order to get reduced or free services from some clinics, they use a sliding fee scale based on your income.When they use a sliding fee scale, the 100% rate can be different than 100%. In those cases, using for example a 200% federal poverty level, you will only need double the 100% number listed above to 200%.
