The mission of the Lake Superior Community Health Center is to improve access, particularly financial access, to primary health care through the provision of clinical care, health education, screening, assisted referrals and advocacy.

Sliding Fee Scale
If you do not have health or dental insurance or if your insurance does not cover certain services, we may be able to base your payment on a sliding fee scale. The scale is based upon household size and income.
You will be asked to provide proof of household income. Proof of income is ordinarily your income tax form 1040 from the last tax year. If proof of income is not provided, you will be billed for the full amount of the services provided. If you have not filed an income tax return, provide a copy of all W-2's, Social Security or pension statements, pay stubs, and any other income received by your household.
Clients will need to pay a fee prior to obtaining most medical or dental services. Clients are expected to pay something towards the cost of their care. Clients may call our billing department with questions or concerns about payments.
If your income and household size does not fall within the sliding fee scale health or dental care guidelines below, you will be billed for the full amount of the services provided.

These numbers above represent 100% of the Federal Poverty Rate. In order to get reduced or free services from some clinics, they use a sliding fee scale based on your income.When they use a sliding fee scale, the 100% rate can be different than 100%. In those cases, using for example a 200% federal poverty level, you will only need double the 100% number listed above to 200%.

My son Alex Garavello Stabs received a letter from the Minnesota Sure health co pay help with his medical bills in 2018. However he does not have a card with the number of his enrollment in this program because he was eligible , he was granted help with having his "co-pay" covered by the Mn Sure Rose was his case worker. I wonder if she is still at the office and if she could please contact me , A
