Faith Community Health provides primary medical care, KwikCare, vision, prescription, and mental health services at affordable income-based rates.
Faith Community Health is a faith-based, non-profit healthcare organization that delivers quality and affordable care to the residents of Stone and Taney Counties. We strive to build a community with access to affordable healthcare services and appropriate healthcare utilization for those in need. Our ministry offers care of the entire person - spiritually, physically, mentally, and emotionally. Our providers recognize that the mind, body, and spirit affect the whole person collectively and a balance of all are essential for health and wellness.
Any individual with income, living in Stone or Taney Counties, qualifies for our services. Fees are determined based on household income and insurance status.
Normal operating hours are:
Registration: Mon - Fri 8:00 AM - 12:00 PM
Medical: Mon, Tue, Thu, Fri 8:00 AM - 5:00 PM
Dental: Mon - Fri 8:00 AM - 5:00 PM
Dispensary: Mon - Fri 8:00 AM - 5:00 PM
Vision: By appointment only

For Households with more than 8 persons, add $4,480 for each additional person.
*Alaska and Hawaii have different rates for HUD federal poverty guidelines.
These numbers above represent 100% of the Federal Poverty Rate. In order to get reduced or free services from some clinics, they use a sliding fee scale based on your income.When they use a sliding fee scale, the 100% rate can be different than 100%. In those cases, using for example a 200% federal poverty level, you will only need double the 100% number listed above to 200%.
