This clinic provides medical and dental needs of our patients through the following services:
Health care from pre-birth through full age, Obstetrics / gynecology, Prenatal, Pediatrics, Well-child visits, Vaccines for Children, Adolescent health, Family Practice, Internal Medicine, Geriatrics

Family dental care, Chronic illness treatment through the Chronic Care Model which includes:
Diabetes care, Chronic Obstructive Pulmonary Disease (COPD), Cardio-vascular disease, Laboratory Services, Referral for Mental Health Services, Nutrition Care, Pregnancy testing, Social services, Cancer screenings, Spanish interpreting, 340B pharmacy discounts, Prescription Assistance Program (free medicines from pharmaceutical companies for those who demonstrate the ability to qualify)
This clinic provides a sliding fee discounts for self-pay patients who meet income eligibility requirements. Self pay patients may qualify for the discount program based on family size and proper documentation of all household income.

For Households with more than 8 persons, add $4,480 for each additional person.

These numbers above represent 100% of the Federal Poverty Rate. In order to get reduced or free services from some clinics, they use a sliding fee scale based on your income.When they use a sliding fee scale, the 100% rate can be different than 100%. In those cases, using for example a 200% federal poverty level, you will only need double the 100% number listed above to 200%.
