About Community Care Center Free Clinic
This clinic operates under a SLIDING SCALE model.This means that it MAY NOT be free depending on your income.You will be required to prove financial need in order to receive free services or services at a reduced cost.In order to get more information on this clinic, click on the icons below. The Community Care Center sees patients Monday through Thursdays, with evening clinics on Monday and Thursday.
The physicians at the Center represent a mix of specialties, allowing most medical problems to be handled in-house.
Basic services and the following, specialty clinical services:
Cardiology
Dental
Dermatology
Ear, Nose and Throat
Endocrinology
Gastroenterology
Gynecology
Neurology
Ophthalmology
Eligibility Requirements:
You must have lived in Forsyth, Stokes, or Davie counties for at least the last three months.
You cannot be eligible for state or federal sponsored healthcare insurance, including Veterans Administration, Medicaid, or Medicare.
You generally cannot be eligible for healthcare insurance through your employer.
To qualify as a new patient:
You must provide all necessary information and meet financial guidelines.
How to apply:
Eligibility screenings are held every Tuesday and Wednesday afternoon from 1:00 pm until 4:00 pm. You do not need an appointment for the interview.

These numbers above represent 100% of the Federal Poverty Rate. In order to get reduced or free services from some clinics, they use a sliding fee scale based on your income.When they use a sliding fee scale, the 100% rate can be different than 100%. In those cases, using for example a 200% federal poverty level, you will only need double the 100% number listed above to 200%.
