No Walk-ins (After 5:00pm appointments are available on Monday & Wednesday for established patients)
Languages Spoken: English and Spanish (Others as needed)
Eligibility: Accepts a variety of health insurance plans, including
Medicare and Medicaid. Provide proof of income for reduced cost.
Cost: Free or sliding scale (Forsyth County Residents only) and
insurance
Services: Chronic Condition Care, Acute Care, Physicals,
Immunizations, Primary Care, Some Health Education, Pharmacy, Lab,
and X-ray. For more information about specialized services please
