Sliding Fee Scale Program:
The Sliding Fee Discount Scale for health care services to patients who reside in the service area defined as Anson, Richmond, Montgomery, and Union counties in North Carolina and part of Chesterfield County in South Carolina.
Eligibility documentation:
Last 30 days proof of income
Previous year Federal income tax return
Bank statements
Unemployment verification
Court documents: citing child support or alimony awarded by the court.
Most recent award letter from Social Security or Veterans Administration, local social services indicating benefit amount.
**Verification of Sliding Fee Scale is required every six months.**
A current sliding fee card or insurance card is required at each visit and must be presented.

In order to get reduced or free services from some clinics, they use a sliding fee scale based on your income.When they use a sliding fee scale, the 100% rate can be different than 100%. In those cases, using for example a 200% federal poverty level, you will only need double the 100% number listed above to 200%.
