This health center is open to all patients with and without insurance. This health center accepts most insurance and will either file claims for you or help with the completion of claims when needed.
This health center offers a sliding fee scale to qualified patients who are uninsured or underinsured. To see if you qualify, please complete the application and bring it to your first visit at the health center along with proof of income and your last tax return, the sliding fee application will help determine cost for services based on household size and income.

In order to get reduced or free services from some clinics, they use a sliding fee scale based on your income.When they use a sliding fee scale, the 100% rate can be different than 100%. In those cases, using for example a 200% federal poverty level, you will only need double the 100% number listed above to 200%.
