As a Federally Qualified Health Center (FQHC) , each of our 4 clinics currently has a minimum nominal fee of $35.00 ( subject to change without advanced or written notice) for an office visit. We continue to offer a sliding fee scale for uninsured, underinsured, and insured patients in accordance with applicable federal guidelines/regulations.

Affordable Care Act: Our certified ACA counselors will help you with your insurance application on Healthcare.gov.
Medication Assistance Program: Meet with our medication assistant program coordinator to apply for medication assistance to obtain your medication.
Patient Portal: A Patient Portal is a secure online website that gives you convenient 24-hour access to your personal health information and medical records.
Referral Coordination: When your provider refers you to see a specialist or to get a specialty test our Referral Specialist will help coordinate your appointment with you up to but not limited to the following

Self Payment, based on a sliding fee scale*
We offer a sliding fee discount program. Based on your income and household size we are able to apply a discount to your visit. On the first visit to the clinic, please bring proof of your income and family size to help us determine your discount

For Households with more than 8 persons, add $4,480 for each additional person.
*Alaska and Hawaii have different rates for HUD federal poverty guidelines.
These numbers above represent 100% of the Federal Poverty Rate. In order to get reduced or free services from some clinics, they use a sliding fee scale based on your income.When they use a sliding fee scale, the 100% rate can be different than 100%. In those cases, using for example a 200% federal poverty level, you will only need double the 100% number listed above to 200%.
