Fairfield Community Health Center is a Federally Qualified Health Center or FQHC. FQHC's are community-based and patient-directed organizations that serve everyone, especially populations with limited access to health care.

Monday thru Thursday from 7:30 am to 4:30 pm
Friday from 8 am to 3 pm.
The Health Center is closed everyday from 11:45 - 12:45.
Fairfield Community Health Center accepts Medicaid, Medicare, CareSource and Molina. The Health Center also accepts self-pay on a sliding fee scale which is based on proof of income and family size.
The Sliding Fee Discount Program is designed to help individuals who cannot afford health care. A person may pay as little as the minimum co-pay of $25.00 a visit, however this may increase depending on the percentage of discount a person is eligible to receive.

In order to get reduced or free services from some clinics, they use a sliding fee scale based on your income.When they use a sliding fee scale, the 100% rate can be different than 100%. In those cases, using for example a 200% federal poverty level, you will only need double the 100% number listed above to 200%.
