This facility is a place where children, families and other adults in the community can come for their medical care. Where they treating people of all ages, the Lincoln Health Center staff consists of a medical doctor and a family nurse practitioner, along with medical assistants and office staff.
Monday, Friday: 8 a.m. – 5 p.m.
Wednesday: 11 a.m. – 7 p.m.
Patients will be seen with or without health insurance, and they offer a sliding fee schedule based on gross monthly income. Clients with insurance, insurance companies can be billed. You can receive assistance with the application process if you qualify for Oregon Health Plan (OHP), FHIAP, or FPEP.

In order to get reduced or free services from some clinics, they use a sliding fee scale based on your income.When they use a sliding fee scale, the 100% rate can be different than 100%. In those cases, using for example a 200% federal poverty level, you will only need double the 100% number listed above to 200%.
