To be eligible, patients must be either uninsured OR have Medicaid OR be within 250% of the FPL.

Licensed professionals to provide to following services:
Regular check-ups (i.e. sugar, diet, blood pressure, etc.)
Referrals (in case of serious medical cases)
Counseling (i.e. mental health, family, etc.)
