Community Health Center Offering Sliding Fee Scale Treatment. This is not a free clinic.
Sliding Fee Program
Our Sliding Fee Program offers a discounted rate on medical and dental services offered at our facilities. A patient can be eligible for this program if they do not have insurance or are under-insured. To qualify for this program you must call for an appointment to meet with one of our Patient Benefit Coordinators at 717-299-6371 and bring the following:
Proof of Identification: photo ID, such as driver's license, state ID, passport
If employed:
1040 Form (income tax return), if not available, please provide last year's W2 along with 2-4 most recent pay stubs, OR
Letter from employer on company's letterhead
Proof of every individual's income living in the house
If unemployed:
Proof of Unemployment Benefits, OR
Award Letter from SSI/SSD benefits, OR
Letter of denial from (Department of Public Welfare), OR
Proof of assistance from another non-profit organization [501c (3)] for example: Church, United Way Organization, etc.

These numbers above represent 100% of the Federal Poverty Rate. In order to get reduced or free services from some clinics, they use a sliding fee scale based on your income.When they use a sliding fee scale, the 100% rate can be different than 100%. In those cases, using for example a 200% federal poverty level, you will only need double the 100% number listed above to 200%.
