Our Sliding Fee Program allows for discounts in fees charged for services provided to you. Discounts are based on household income and family size.
For medical services, you may qualify to pay only $60, $40, or $10, with in-house lab work and x-rays provided at no additional cost. Dental bills may be reduced by 33% or 50%.
An individual whose income does not allow for reduction to his/her medical bill will be charged a reduced flat fee of $70 in addition to any lab and x-ray charges if the bill is paid at time of service.
Everyone is eligible to apply for the Sliding Fee program, even those with medical insurance. Once your eligibility is determined, your sliding fee will be in effect for a one year period.
Prescriptions are not covered under the Sliding Fee program, but completing the Sliding Fee application will help determine eligibility for prescription assistance programs.

In order to get reduced or free services from some clinics, they use a sliding fee scale based on your income.When they use a sliding fee scale, the 100% rate can be different than 100%. In those cases, using for example a 200% federal poverty level, you will only need double the 100% number listed above to 200%.
