Santa Fe Family Wellness Center provides Primary Care which includes chronic illness care for illnesses like diabetes, high blood pressure and asthma, acute illness care, preventive medicine, family planning, womens health, wellness physicals and screenings.
They also provide support for a wide range of behavioral health conditions.
Their services include crisis intervention, comprehensive assessments, individual and family counseling, psychiatric consultation and medications, substance abuse programs, educational groups for a variety of problems, and additional community support programs for those with serious, chronic mental health conditions.
They accept Medicaid, Medicare, and private insurance. For the uninsured, a sliding fee scale is available based on income.
Hours:
Primary Care: 8:00 am - 5:00 pm Monday - Friday
Behavioral Health: 8:00 am - 7:00 pm Monday - Friday
