We are open for Primary Patient Care EVERY Tuesday at 5 PM. (Except Holidays). We no longer have a 3- county restriction, so if you are in need of the services we provide, do contact us.
Please contact us before coming the first time to coordinate care.
We do not schedule appointments.
OTHER Services, Beyond Primary Care:
We have some other services beyond Primary Care. These services change each week. You can get these services after you see a provider or you may be asked to come to the clinic only for these services. The typical monthly schedule is:
~Physical Therapist: 2nd and 4th Tuesdays
~Diabetes Educator: 3rd Tuesdays
~Dietician: once a month or as scheduled
~Orthopedics, Opthalmology and Dermatolgy: by Referral
(Unfortunately, we are unable to offer psychiatric services)
Our location is 1035 North Main Street in Rice Lake, WI. between the sites of the old hospital and the old clinic on the far North end of Main St. Our office is located on the ground floor, please enter from lakeside (Lakeview Drive). The sign over the door says Clubhouse.

For Households with more than 8 persons, add $4,480 for each additional person.
*Alaska and Hawaii have different rates for HUD federal poverty guidelines.
These numbers above represent 100% of the Federal Poverty Rate. In order to get reduced or free services from some clinics, they use a sliding fee scale based on your income.When they use a sliding fee scale, the 100% rate can be different than 100%. In those cases, using for example a 200% federal poverty level, you will only need double the 100% number listed above to 200%.
