HopeHealth is a nonprofit organization that comprises a Federally Qualified Health Center (also known as a community health center), three specialty infectious disease practices, a child advocacy center and a wellness center. Among our 10 community health centers, our services include primary and preventive care, chronic disease management, pediatrics, internal medicine, infectious diseases, behavioral health, and medical massage therapy.
All patients at HopeHealth are responsible for paying for their care; there are programs in place to ensure this care is affordable for everyone. Our providers and staff are all paid employees, and we see patients primarily by appointment.
We take pride in treating all our patients with respect and dignity, regardless of insurance status. You can set an appointment to be seen by filling out our new patient intake application or contact the site nearest you. In order to determine whether you qualify for our sliding fee discount program, you must be prepared to provide proof of income at your first appointment. Upon setting your appointment, our patient account representatives will provide a list of what you will need to bring. If you do not bring what we request, we will be unable to enroll you in the discount program and you will be responsible for paying the full cost of care.
Since this is a sliding fee scale clinic, we have provided the Federal Poverty Guidelines below. Visit the HopeHealth Lake City website listed above to see what the level is needed for free care.

*Alaska and Hawaii have different rates for HUD federal poverty guidelines.
These numbers above represent 100% of the Federal Poverty Rate. In order to get reduced or free services from some clinics, they use a sliding fee scale based on your income. When they use a sliding fee scale, the 100% rate can be different than 100%. In those cases, using for example a 200% federal poverty level, you will only need double the 100% number listed above to 200%.
