- CHC will file all Medicare and Medicaid Claims.
- CHC will file most in-state private insurance claims.
- Patients are responsible for paying deductibles and co-payment at the time of the visit.
- Low-income patients who want discounted services must apply for these services in advance and present proof or statement of income, or sign a statement declaring their status.
- Patients on the discount program must pay the minimum fee. (Currently $18)
- Patients are expected to pay in full at the time of visit when CHC is not filing insurance. You will receive a receipt so that you may file your own insurance.
- Patients with a previous unpaid balance must pay for that day's visit and will be encouraged to set up a payment plan for the previous balance.
