APPOINTMENTS & WALK-INS
We require patients to call or come into the office to schedule an appointment. The Health Center Care team will make every effort to ensure your urgent needs are met for established patients who require same day appointments.
Patients are encouraged to keep their appointments or notify the Center 24 hours in advance. If you are unable to keep your appointment please call and let us know ahead of time. Bring all medications (pill bottles), immunization records and insurance cards to every visit.


PATIENTS WITH INSURANCE
Patients with insurance are required to present their insurance card at each visit. Coverage will be verified before each office visit. Patients must pay the co-payment or co-insurance amount as required by their insurance company. Claims rejected by the insurance company wil be billed directly to the patient.

    Nominal Fee: $30.00 (applies to patients <100% of poverty level.)

    I've been feeling really bad for two weeks. I'm getting rashes and swelling on my feet, hands, neck face. I'm getting weaker everyday. My legs feel so heavy and they get numb and tingly. I have been to this clinic.I have diabetes. I had women's health issues. I paid based on my income (sliding-scale). The staff was friendly. I waited less than 30 minutes To Whom it may concern; I'm writing/emailing this note just to let you know about my care at FCF. I have been coming to FCF for numerous years and have received good sound medical care. All the staff has been professional and informative and friendly. I realize friendliness is not part of their duties; but nevertheless, they are friendly also. Most of my family and a lot of people assume because its a clinic and not a doctor's office, that the care is less than. It is not!! I get excellent care from the staff and doctors at FCF. I want to especially mention Dr. Shantae Jenkins and Janette, and Holly from the Hollywood Branch. We miss you Dr. J. and Janette; and Natasha Chatman and Melody Brown.
